World Mental Health Day – Young People & Mental Health

Today is World Mental Health Day. This year the World Health Organization has decided that the theme is young people and mental health in a changing world. So here’s my post all about young people and mental health:

What is a young person?
A young person is any person aged between thirteen and twenty one years old. They are in the adolescence stage of life:

Adolescence
The stage of adolescence or the teenage years starts around 12-13 years old and lasts until around 18-20 years old. The end of childhood and beginning of adolescence is marked by the start of puberty.

In adolescence your body and brain are going through massive changes. These changes mean that you need more sleep and may well be found sleeping in till midday or later.

In the body, high levels of hormones rage creating physical changes, increasing emotional intensity and a fierce need for independence. The emotional intensity explains why you are more likely to be rebellious in your thinking and actions as an adolescent.

If you’re a male physical changes include: body hair grows (public, underarm, facial & legs), voice breaks – becoming deeper, Adam’s apple becomes prominent, acne, penis growth, lowering of testis lower, growing taller and broader.

If you’re female physical changes include: growth of breasts, menstrual cycle, body hair grows (public, underarm & legs), acne, weight gain, change of body shape and growing taller.

In the brain, hormones make you more likely to act on impulse and take risks. This explains why you are more likely to try alcohol and/or drugs in adolescence.

Both the body and brain make you aware of those you feel sexually attracted to. You notice them. You want to get to know them and ultimately you want to have sex with them. You may start to have sexual and/or romantic relationships.

Your thinking is egocentric – you are only or mostly concerned about you. This egocentric thinking explains a lot of your thinking and behaviour including why you are so concerned about how you look.

Puberty takes around 4 years from the early signs to completion. As well as all the puberty changes, you have the pressures of high school including peer groups and exams.

Why focus on young people?
These statistics explain why:

  • 20% of adolescents may experience a mental health problem in any given year.
  • 50% of mental health problems are established by age 14 and 75% by age 24.
  • 10% of children and young people (aged 5-16 years) have a clinically diagnosable mental problem, yet 70% of children and adolescents who experience mental health problems have not had appropriate interventions at a sufficiently early age.

What can we do to support young people with mental health problems?
First teach them about resilience and how to increase resilience:

Resilience or emotional resilience is our ability to deal with adverse events in life that cause pressure or stress. I like to think of resilience as being like water in a well:

We only have so much resilience within us, like there is only so much water in a well. Adverse events cause us to use our resilience water by the bucket loads. But there is good news.

We can make it rain to add more resilience water to the well or just top it up a little at any time. We can do this by: Taking a Break, Relaxation, undertaking Hobbies & Interests, Spending Time in Nature, Practicing Meditation or Mindfulness, Spending Time with Family / Friends / Animals and Listening to Music.

Second support young people in a variety of ways including:

First educate yourself around mental illness.

Second: You must look after yourself. You can’t support anyone else if you are not well physically, mentally or emotionally.

On to the practical advice to support someone with mental illness:

  • Stay in contact with them. Ask them how they would like you to keep in contact. Some may may prefer phoning or seeing; whereas others may prefer messaging or texting.
  • Unconditional love and care. Let them know that you love them unconditionally and care for them deeply. Don’t have any expectation that they will reciprocate.
  • Listen to what they say and don’t assume anything.
  • Ensure that there are no distractions when you are with them or on the phone to them. Put your phone on silent and make sure any loud children are pets are out of the way (if you have them).
  • Offer practical support. Go shopping for them or with them, help them to clean, cook them a meal. Whatever it is that they need. They maybe resistant to the idea of practical help, so reassure them that you are happy to help and that you know they’d do it for you.
  • Remind them to take their medication or when appointments are due. People with mental illness tend to have poor short term memories.
  • Ask them about their appetite and diet. If they have an appetite but are struggling to make anything (due to lack of energy and/or motivation), find out what there favourite meal is and cook it for them.
  • Offer distracting activities. Distracting activities that you can both do together can give someone a break from their own critical inner of voice. The activities can be something as simple as a walk around the park. Make sure you are always led by the person with mental illness though. If they say that they are too unwell or tired to do the activity, don’t take it personally. And certainly don’t judge them or take offence.
  • Help them access support. This could include going with them to GP, counselling sessions or mental health service appointments. Offer to sit in appointments with them, but let them know that it’s okay if they want to be seen alone.
  • Be understanding. Someone with mental illness may cancel plans at the last minute. You may arrive at their house to find it messy and them unclean. Don’t take it personally, let them know that you understand and ask if there is anyway that you can help.
  • Be patient. Like any illness, mental illness takes the right treatment, the right support and time for them to start to feel better.
  • Limit questions and time spent with them, if you feel they are exhausted and need to rest. You’ll be able to spot if they need to rest by: pulling on their hair, forgetting what you’ve said to them, being very slow to respond, unable to think of words, dropping of their head, shuffling of feet and other body language people use when they look like they are about to drop off to sleep.
  • Be aware of your own body language and theirs. Try and display open body language and avoid mirroring.
  • Try not to give advice, as often it is unrealistic and unhelpful. For example never advise someone with depression to exercise more or have an healthier diet. This person has probably used all of their energy and motivation to get out of the bed. This single action has left them more exhausted than they have ever felt in their life. So advising them to exercise, eat an healthier diet or make big changes to their life will seem unachievable and may come across as if you are blaming them for their depression.
  • Sign-post them to useful resources.

The Teenage Brain

I’ve worked with teenagers, or adolescents or using the term I prefer young people for most of my career.

I’m often asked the same questions about young people, such as: Why do they take risks? Why can’t they always think about the potential consequences of their actions? Why can’t they see the perspectives of others? Why are they so concerned about what their peers think of them?

This great TED Talk video explains more about young people’s brain and its development:

Although this video is enlightening, it shows that neuroscience is in its infancy. Our understanding of probably the most the most important organ in the body is extremely limited.

I wish the speaker had gone into further details about young people’s behaviour, so that the questions above were more fully answered for those that have them.

Wonderful Websites – General Health

wonderful-websites-image This is the first blog post in a series titled Wonderful Websites. This post focuses on the most wonderful websites for general health.

1. NHS Choices is crammed with health information. Information on health conditions, treatments, medications, public health issues, carers right, services, practically everything you could ever want to know about health and healthcare in the UK. Delivered by the NHS you can be assured of correct information that is up to date and based on research and evidence.

It has some great online tools such as: BMI Calculator, Mood self-assessment, Sleep self-assessment and many more.

2. Patient.info has a A-Z list of articles written by doctors, information videos, a symptom checker and an active online community. On more than one occasion while at work in the NHS, I’ve seen doctors checking this valuable website.

3. NetDoctor is similar to Patient.Info, but with more of a focus on healthy living and healthy eating.

4. eMC (Medicines.org.uk) is a useful website for looking up information about medicines.

I use British National Formulary (BNF) but the medical terminology can be difficult for some from a non-medical/non-nursing background to understand; this is why eMC made it on this list instead of the BNF.

5. Office of National Statistics – Health and Social Care is the best place to find statistical information about health. It is regularly updated based on data from the NHS, Local Authories and other Government Departments.

6. National Institute of Clinical Excellence (NICE) provides pathways, guidelines and advice across the UK for best possible evidence-based healthcare. The guidance they provide is developed by groups of clinical experts who provide experience of what works in practice and by examining and analysing research.

If you want to know what care you or a relative or friend should be receiving and check that it’s high quality visit NICE’s website.

7. Health For Teens I found while doing research for this post. It’s specifically aimed at teenagers and looks appealing. They even create content in consultation with young people. I think the name of the website maybe off-putting to young people though – but this is just my personal opinion.

Book Review: Under The Dome by Stephen King

under-the-dome-stephen-king-book-cover Stephen King is a Writer that I’ve always admired. But to be honest, he’s wrote that many books, I’ve always been unsure where to start.

That was until I watched the TV series Under The Dome, based on King’s two-book story with the same name. The copy of Under The Dome that I’m reviewing is one where the two books have been combined into one and therefore has the full story from start to finish.

My Review
Under The Dome is the masterpiece novel Stephen King. It literally took over my life for a good few weeks. At every available opportunity, I’ve found myself picking it up and reading more.

Under The Dome starts when an invisible dome descends on the sock-shaped town of Chester’s Mill, Maine in the USA. The dome is almost impenetrable, only letting through small amounts of air and water.

When the dome comes down it slices off the hand of a woman gardening. It slices a small aeroplane in half. A few cars crash into the dome, which explode on impact. The gardener, aeroplane pilot and trainee, and car drivers all die.

But for the people of Chester’s Mill this is just the start of their problems and things are going to get a hell of a lot worse.

Under The Dome has a full town cast of characters. Here are some of the characters, in alphabetical order:

  • Andrea Grinnell – local politician (Third Selectman) and addicted to prescription painkillers.
  • Andy Sanders – Local politician (First Selectman) and Pharmacist. Owner of the only drug store, which would have closed years ago, if it wasn’t for the help of Jim Rennie.
  • Colonel James Cox – In charge of the military outside of the dome.
  • Dale Barbara (Barbie) – A Iraq army veteran.
  • Duke Perkins – local Police Chief. That is until he meets his demise and is replaced by Peter Randolph.
  • Jim Rennie (Big Jim) – Local politician (Second Selectman) and a used car salesman. He also has a secret illegal business of making, selling and shipping methamphetamine.
  • Joseph McClatchey (Scarecrow Joe) – a very clever teenager. He is often with his two friends (Norrie Calvert & Benny Drake) throughout the book.
  • Julia Shumway – Owner, writer and editor of Democrat Chester’s Mill local newspaper. Has a Corgi dog named Horace.
  • Junior Rennie – Jim’s son. Revealed to have a brain tumour that nobody knows about that influences his thinking and behaviour.
  • Ollie Dinsmore – The boy who looses everyone and everything, but manages to survive.
  • Piper Libby – A Reverend who doesn’t believe in God and lives with her dog whom she loves dearly.
  • Rommie Burpee – Owner of Burpee’s Department Store.
  • Rusty (Physicians Assistant) & Linda (Police Officer) Everett – Rusty’s character is brilliant. He sees the truth because he questions everything and follows his gut feeling. But with a wife and two children he is constantly torn between doing the right thing and protecting his family.
  • Samantha (Sammy) Bushey – A girl with a difficult life, one that’s going to get more difficult with the dome in place and will lead to tragic consequences.
  • Thurston Marshall & Carolyn Sturges – Out-of-towners.

I know that I have probably missed out someone’s favourite character. If I have and you want to let others know about your favourite character, leave a comment below, giving the character’s name and a brief description.

So what actually happens in the just over a week that the dome is in place? The answer plenty including: manipulation, lies, abuse of power, crimes – looting, rape (this scene was particularly disturbing and traumatic to read, but none the less extremely well written) and murder, false allegations, the attempted cover-up of meth labs and propane use/storage, the threat of diminishing resources – people don’t know how long the dome will be in place for and how people respond to this threat, a major explosion and fire fuelled by the propane and an abundance of death.

The TV series was mediocre. I am pleased to say that the book far exceeds the TV series, being extraordinary. In this letter King explains that in the TV series the concept of the dome is the same, but the Writers have re-imagined the plot and some aspects of the characters. He states that he sees the TV series as playing out in an alternate reality.

The Under The Dome concept is brilliant. The description is superb. The characters are have been well developed and are interesting. The pacing is terrifically fast meaning that the book grips you from the first page to the very last (it’s a total of 877 pages long). Overall Under The Dome is exceptionally well written, with not a single word wasted. Well done King.

I would go as far as saying that in writing Under The Dome King has reached his pinnacle, but I don’t feel I can say this as I haven’t read any other of his works, yet.

The Story Behind The Story
Stephen King had the idea for Under The Dome over twenty-five years ago. But every time he tried to write it, he didn’t feel that he could do the story justice. Then he was involved in a car crash, where he nearly died. For a while after the accident he thought that he might never write again. When King did start writing again, albeit more slowly than before the accident, he decided to start and complete Under The Dome as he didn’t want to die with an unfinished manuscript in his desk draw.
